Incoming and current students are strongly encouraged to attend the sports physicals night offered at BCHS. (Grade school athletes - bring your CYO form and be ready for next year.)

Sports physicals for the 2010-11 school year will be held at Bishop Chatard on Tuesday, June 1, 2010 from 6:00 P.M.-8:00 PM in the varsity gym. The physicals are given by St. Vincent Sports Medicine under the direction of Jon Grant, LAT, ATC & Jason Morgan, LAT, ATC. The cost of the physicals will be $20.00 with $10.00 coming back to BCHS Athletics. Only checks (payable to Bishop Chatard) or cash will be accepted.

FOR PHYSICAL NIGHT: Please download the 5 necessary forms below, and have them completed in their entirety (with signatures) when you arrive.

If you have any questions, please contact the Athletic office, 251-1451 ext 2249. (The forms can also be picked up at the Athletic Office at any time.)

PHYSICALS MUST BE GIVEN AFTER MAY 1ST IN ORDER TO BE CONSIDERED FOR THE 2010-11 SCHOOL YEAR.

Important: ALL PHYSICALS FORMS NEED TO BE COMPLETED AND FILED IN THE OFFICE BEFORE PARTICIPATION IN SUMMER AND SEASONAL CONDITIONING!!

We REQUIRE the following forms to be turned in before the student can participate in conditioning and practice for each respective sport:
